"Made a little mistake and suddenly things went haywire?"
AKA My entire experience playing Fire Emblem Fates: Conquest?
I swear my entire time playing that game has been putting an hour into a chapter then making one minor slip-up and immediately losing a character. Accidentally used Pair Up instead of Shelter? There goes Elise. Moved a character out of the projected danger zone, not realizing that moving them expanded the enemies' range? Bye-bye, Nyx. Mashed through a menu too fast and froze a maid instead of the Adventurer boss? Looks like Beruka's getting shot right out of the sky. I've come to realize that the game doesn't really require perfect strategy, at least on Normal, but it almost always requires perfect execution of whatever strategy you choose
Aside from that, I don't even know where to begin. Video games are such a win-scenario/lose-scenario medium that things go haywire all the time just as often because of a minor mistake as a large one. I could probably name an example of this happening at least once in every video game I've ever played.

Once upon a time when I was playing Pokémon Colosseum, I missed one of the shadow Pokémon I was supposed to snag. Normally, if you miss a shadow Pokémon you get a second chance at them in another area of the game after your initial encounter, and capturing all of the shadow Pokémon is required to beat the game. But, for some reason, the shadow Pokémon I missed -- I want to say it was a Remoraid? --didn't respawn at the second chance location. It must have been a rare glitch, because no matter what I did I couldn't progress. Eventually I had to start all over. Of course, as one of the only people who truly loved Pokémon Colosseum, and this being over ten years ago when I had a fraction of the games I have now, it wasn't too painful a restart, but it was still frustrating.
Side note, I really want another game in the Colosseum/XD series. And Pokémon Ranger. And Detective Pikachu in English.

Pokemon Colosseum is the best! Might still be my favorite Pokemon game. I remember crying when Gamestop lost my preorder for it... And crying every time Miror B's Ludicolo wiped my team... Apparently this game made me cry a lot. But I don't think you need all the shadow Pokemon to beat the game? I mean, you can't even catch some of them until after the end of the main story. It's been a couple of years since I replayed it but I don't remember that being a requirement.

I agree with KerfMerf, pretty sure you don't need all the shadow pokemon to beat it. 6-year-old me definitely didn't catch them all but I was still able to beat the game. Maybe there was another bug that just stopped you from progressing entirely?

Ugh, that sucks. One time I was grinding up for Red in HG, closed my DS for a second for whatever reason, and when I flipped my top the damn thing just would NOT come out of sleep mode. I had to eventually reset, and lost an hour or two of grinding progress :<

Was playing X and Y, and with the new changes it was supposedly easier to breed and find shinies. Um, so, I got ahold of a Japanese ditto with the IVs that I wanted, and started the long process of trying to breed myself a shiny Squirtle (my favorite Pokemon) with at least 5 perfect IVs.
So, every day for about 5 months I would breed for about 3-4 hours, usually during the slack-off classes I had during school because it was my senior year. Well, I finally got my perfect little Squirtle, but the bell rang to go the next class. I put my 3DS in my hoodie pocket and it promptly slid out and hit the floor. The cartridge popped out, and now I don't play Pokemon anymore.
So, every day for about 5 months I would breed for about 3-4 hours, usually during the slack-off classes I had during school because it was my senior year. I was starting up the first ratchet and clank (I was almost done the game at the time) and instead of picking load game I went to new game by accident. I selected my save file and the game went the beginning scenes. The game deleted my old file and all I had was this new untouched save. It took me forever to pick the game back up again after that.

I nuzlocked Alpha Sapphire when it came out. It was going pretty well, I started with my lovely treecko, formed a team and everything went relative smoothly.... suspiciously smoothly. I got my mega ring, the megastone for sceptile, and went to fight Kyogre. I was like "lol, I'm resistant to all your water type moves, so I'll just add salt to the wound and make it a 4 times resistance". I mega evolved sceptile.... but Kyogre was faster... Kyogre used ICE BEAM... It's was Super (4 times) efective. And that's how I lost my partner to one careless little mistake.

Yes, the flaming helicopter in the "Mission Complete" image was my helicopter, the one I was supposed to get safely out of there. The game afterward shat itself, this is what happened in order:
1)The helicopter is destroyed, but apparently falls in the rescue zone.
2)"Mission Complete" is displayed while the helicopter burns in the background
3)I get a "MIssion failed" Screen
4)I get a victory screen congratulating me for saving the soldiers and a list with the soldiers saved
5)The list is empty
6)I get an achievement for rescuing a soldier
7)The campaign resumes and never acknowledges that this event ever happened.

During my first playthrough of Tomb Raider: Underworld, I got to the South American level and pretty much dicked myself over by not knowing how to solve the puzzle. To avoid as many spoilers as possible, there is this puzzle near the end of the level where you have to close the mouths of these serpent statues to get this relic at the bottom.

If you grab on to the handle you can close the mouth, but you can use the ring on the bottom of the mouth to climb back up, which will open the mouth and pour "water" again.
I made it to the bottom and opened up the hole I need to get to. The problem is that the hole was off the ground and I could not figure out how to get to it. So I start to climb up, using the only way I know how, the rings on the statues, which starts to fill up the room. The good news is that I managed to get to the hole by swinging to the pillars next to the hole. The bad news is that the cutscene that plays after grabbing the story related relic auto saved my game in the middle of raising "water." When the cutscene ended, Lara was now drowning and died. The game then reloads my save, and Lara goes back to drowning.
Luckily, I have been saving every so obtain because the treasure trophy is known to glitch and a guide pretty much told me to do so. My last manual save was at the other end of a large tunnel right before the room. If I hadn't been going after a glitched trophy, I would have had to restart the entire game, and I don't know if I could have done that.

I completed the game and met the President who thought I was awesome and he wanted me to high 5 him.
Me being me, I pressed the wrong button and shot him...... which triggered the bad ending

I was playing Just Cause and was doing a mission where you had to blow up a gas station. This was obviously a side mission thank god but when I got there problems started. I threw a grenade into the station and it didn't blow up. I tried again, nothing. Then I realized I could walk through the gas station as the model was not solid. So for some reason, the gas station couldn't be blown up as I could walk through it. I tried doing everything like using remote C4 and timed C4 but nothing worked so I ended up going back to a save which sucked because I had to go and do the Rioja settlement takeover again that I did earlier. I didn't have my phone to take a picture of it but it was pretty crap that it had to come to restarting.
EDIT: I just realized that the bomb missions are absolutely broken and cannot be completed because of the same bug over and over again. What I didn't know was that if you go back to the mission giver, he gives you a totally different mission. But the side missions are just horrid so I'm only going to do Main missions from now on.

Superhot VR. I killed all enemies on the last level and instead of touching the triangle to go back to the main room, I decided to try to shoot myself in my mouth with a gun that I was sure was out of bullets. I killed myself and had to redo the whole thing.

I just finished Shadowrun for SNES last night. Early on in my playthrough I was stuck in a situation where I didn't have a strong enough weapon to do any damage to the enemies I needed to kill to progress. Unfortunately I had wasted cash earlier on hiring temporary team members and buying junk I didn't really need.
Long story short, I had to grind for several hours to earn money 10 or 20 units at a time in order to buy a shotgun that cost 15k. Yikes!

Not sure if this counts as a "little" mistake, but I didn't think my actions would have repercussions. I'm currently playing through Fallout 4 and was beginning to do all the Vault-Tec quests. Because of my best friend, Mr. OCD, I deleted every item in the entire vault to start from scratch. Needless to say the Vault-Tec quest-line in its entirity is broken and I can no longer progress. I also did quite a lot after deleting the items before I realized it was bugged, so I can't just go back to an earlier save point before it all. I guess at least I have an empty vault now...

Playing Grid 2, about to win the race. Literaly 15m away from the finish line, I had exited the last drift poorly and lost control on the very last straight of the track. One second I'm playing Grid 2 and the next I'm playing Kerbal Space Program.
Needless to say I lost the race. Although I got a cool screenshot out of it.
